Please call the office to schedule an appointment for your child to receive the flu vaccine at one of our upcoming Flu Vaccine Clinics!

Children ages 5 and younger usually receive the vaccine in the thigh muscle, wearing shorts or a skirt makes this easier. Our flu vaccine consent form should be printed and completed prior to the appointment. It can be accessed from the following link: Flu Vaccine Consent Form.
